Medical Programs
Each case manager assigned to the Community Transition Unit (CTU) seeks to ensure a continuum of care is provided to all incarcerated persons. Unfortunately, many incarcerated persons suffer from chronic and oftentimes fatal illnesses. The harsh reality is the jails are receiving increasing numbers of seriously ill inmates, including many who are at terminally ill.
The Volunteers of America Grace Project has developed a promising approach to providing access to quality transitional medical services and end-of-life care for jail inmates with HIV/AIDS and other life-threatening illnesses. Frequently, people enter the criminal justice system with a history of under-utilization of community health and social services, and co-existing conditions of substance abuse, mental illness, and homelessness.
The CTU has developed a partnership with the Volunteers of America Grace Project. The Grace Project is a promising approach to providing access to quality transitional medical services and end-of-life care for jail inmates with HIV/AIDS and other life-threatening illnesses. Frequently, people enter the criminal justice system with a history of under-utilization of community health and social services, and co-existing conditions of substance abuse, mental illness, and homelessness.
The Grace Project utilizes a case management model that reaches inmates soon after a chronic or terminal diagnosis has been made. Grace case mangers develop an individualized service and comprehensive plan that supports the continuity of care and promotes quality transitional and end-of-life care. The Grace Project and the CTU also collaborate with a number of other service providers – such as the Medical Services Bureau and community based organizations -- to facilitate quality transitional and end-of-life services to inmates.
